Real time reporting on active sessions

Is there any way of getting real time (or nr real time) stats for active calls coming from specific ip address(es)? Either via SNMP, PerfMon,WMI, anything in fact.
This is not alerting but a way I can go, yup, we currently have X number of active calls from <ip_address)
Any help will be apprecitated
